چکیده

Introduction Acidosis is a frequent disorder among critically ill patients. When patient compensatory responses fail to restore a normal pH, administration of sodium bicarbonate (NaHCO3) or renal replacement therapy may be required. Intravenous NaHCO3 increases plasma Strong Ion Difference ([SID] = [Na] + [K] [Cl]) and HCO3 concentration by raising Na concentration. Although effective, this treatment is not devoid of complications, such as hypernatremia, hyperosmolarity and fluid overloading[1]. Selective chloride (Cl) removal, by increasing SID in an alternative way, may allow a rapid correction of acidosis without altering plasma osmolality and Na concentration.

An Innovative Technique For Extracorporeal Carbon Dioxide Removal Featuring An Electrodialysis Unit: An In-Vitro Experiment

Introduction Acidification of blood entering the membrane lung (ML) converts bicarbonate ions into dissolved gaseous CO2, increasing the pCO2 transmembrane gradient and thus the extracorporeal carbon dioxide removal (ECCO2R) [1]. Extracorporeal blood acidification has previously been achieved by infusion of lactic acid, which proved to be effective in rising ECCO2R but determined a mild increas...

Low-flow CO2 removal integrated into a renal-replacement circuit can reduce acidosis and decrease vasopressor requirements

INTRODUCTION Lung-protective ventilation in patients with ARDS and multiorgan failure, including renal failure, is often paralleled with a combined respiratory and metabolic acidosis. We assessed the effectiveness of a hollow-fiber gas exchanger integrated into a conventional renal-replacement circuit on CO₂ removal, acidosis, and hemodynamics. METHODS In ten ventilated critically ill patient...

Pumpless extracorporeal lung assist for the treatment of severe, refractory status asthmaticus.

BACKGROUND Until recently, the only available lung-protective treatment option for carbon dioxide removal due to severe, refractory status asthmaticus has been extracorporeal pump-driven membrane oxygenation (ECMO). Pumpless extracorporeal lung assist (pECLA) may serve as an alternative therapy for these patients. CASE REPORT A 42-year-old woman presented with an acute exacerbation of asthma ...

Bronchial fistulae in ARDS patients: management with an extracorporeal lung assist device.

Patients with bronchial tree lesions feature, in particular, a high risk for developing bronchial fistulae after surgical repair when the clinical situation is complicated by acute lung injury (ALI)/acute respiratory distress syndrome (ARDS) and mechanical ventilation is needed. The current authors hypothesised that extracorporeal carbon dioxide removal would significantly decrease inspiratory ...

Veno-venous extracorporeal CO2 removal for the treatment of severe respiratory acidosis

We read with interest the article by Karagiannidis and colleagues reporting the effects of extracorporeal CO2 removal (ECCO2R) in a pig model of severe respiratory acidosis [1]. However, their conclusion that blood flow rates between 750 and 1,000 ml/minute are necessary to correct severe acidosis using ECCO2R may have been biased by limitations in experimental methodology. Firstly, the authors...
